CWP ELIGIBILITY REQUIREMENTS & REVIEW PROCESS
Certified Wellness Practitioner (CWP) application must be submitted through an online form and applications are reviewed once all materials and full payment are received. Applicants will then be notified of their acceptance status via e-mail within 4-5 weeks. Approved applicants will receive an official certificate in the mail.
To be eligible to submit a CWP application, you must meet one of the following educational and work experience pairings at the time of application submission:
All CWP applications must include the following:
1. OFFICIAL transcripts documenting your academic preparation, degree(s) and educational background. Transcripts from each degree completed are required. If you received your highest degree from an institution outside the United States,
please apply for a WES credential evaluation (additional fee through WES) and submit an official credential evaluation report in lieu of transcripts. Send official transcripts or official credential evaluation report to:

2. Your résumé/curriculum vitae providing evidence of full-time, post-degree work experience in the wellness/health promotion field where wellness/health promotion is a direct responsibility in your job description. Volunteer, leadership, and continuing education experiences should also be included in a separate category within the résumé /CV. Upload to the online application form.
3. Application Fee of $499* ($50 non-refundable) payable to the National Wellness Institute. Payment may be made online after submitting the online application form. If “pay by check” is selected, application will not be reviewed until payment is received.
Upon approval, the term of certification is five years and automatically includes a five-year membership in the National Wellness Institute (NWI) at no additional charge (currently, a $745 value). If your application is not approved, you will receive a refund of $449.
4. Completed online application form (below). The application begins with the Wellness Promotion Competencies Form. Please note that if you need more time to complete this form, you have a "save for later" option. Draw selectively from your academic preparation, work experience, and continuing education to provide meaningful evidence that collectively you demonstrate proficiency in the required competency domains. When you submit this form, you will be directed to the rest of the online application before being sent on to the payment page.
